Sage Elle Hapke is the Associate Director of Complex Care at the San Francisco Community Health Center, where she has created and overseen programming that connects transgender individuals to health and wellness services, including behavioral health, primary care, gender-affirming care, HIV prevention and treatment, substance use treatment, permanent supportive housing, and reentry support. Sage is a woman of trans experience and has supported transgender community members in San Francisco for more than a decade. She serves as the Board Treasurer for the Castro Country Club, a sober community center. Sage previously helped open the nation’s first transgender homeless shelter, the Taimon Booton Navigation Center, and taught classes at San Quentin State Prison through the Prison University Project. She is a Master of Social Work candidate at California State University, East Bay, and holds a Bachelor of Arts in Critical Diversity Studies from the University of San Francisco.
